I think the original adage was beauty is only skin deep but ugliness goes to the bone.

I knew a kid who found skulls genuinely beautiful, he collected animal skulls. He was really fucking weird and I would not be surprised if he had some human skulls somewhere.
I can all but guarantee he’s got at least one.
Always assumed the phrase was referring to your attitude. Beauty is only skin deep is something you say about a handsome asshole or a beautiful cunt. If you look max you are often neglecting a personality. Hince their beauty being only skin deep. Look any deeper into them and you see a cesspool of a personality.
